Housing is health care.

Stable housing is a key social determinant of health. Yet health care systems are not designed to address housing, and homelessness programs are not equipped to care for the complex health needs of people experiencing homelessness. Effective coordination is critical.

Achieving collective impact in housing and health.

Robust intentional collaboration can break down silos and achieve the potential of health through housing, leveraging opportunities afforded by Medicaid, managed care, data sharing, discharge planning and other important strategies. Our health system team is comprised of experts in health, behavioral health, and homelessness, skilled at facilitating sustainable partnerships and interventions grounded in the most impactful national practices.

Harnessing the potential of integrated systems, programs, and partnerships.

  • By empowering people experiencing homelessness, cross-sector providers, systems leaders, and state policymakers, we support our partners to achieve the full benefits of housing as health care. Homebase’s detailed case studies and tools support cross-sector partners to learn from other communities and leverage local potential for lasting impact.

  • Working with government agencies, we help develop and operationalize Medicaid waivers and leverage federal and state resources at the intersection of homelessness and physical and behavioral health. At the state level, we ensure policies, strategic plans, and funding streams are designed to promote best practices. Locally, our hands-on technical assistance forges strong partnerships and effective resource deployment, translating these policies into impact on the ground.

  • Homebase peer-learning models bring together networks of partners to learn from one another and implement best practices in their own communities. Homebase facilitates action-focused planning, from HUD’s national Housing and Health Care (H2) Systems Integration Initiative, to CoC Peer Learning Roundtables on California’s 1115 Medicaid waiver for housing-related supports implementation, to monthly office hours for providers assisting people experiencing homelessness with their Medicaid renewals. 

  • Homebase works directly with communities, systems, and programs to integrate health care and homelessness systems, stand up new street medicine and medical respite/recuperative care programs, and leverage Medicaid and other resources. We leverage action-focused facilitation, best practices and emerging strategies, and human-centered system design to help local and regional health care and housing partners identify shared priorities and develop sustainable relationships, metrics, and innovations.

    Engaging, interactive workshops and direct technical assistance for Managed Care Plans, state and county agencies overseeing health and human services, and other health care partners supports translation across systems, including coordination with homeless and housing systems, Homeless Management Information Systems (HMIS), and Coordinated Entry.
