Connecting for Better Health, Homebase, and Georgetown University’s Massive Data Institute launched the CoP to help California CoCs, MediCal Managed Care Plans, and their partners overcome data sharing challenges that prevent shared clients experiencing homelessness from accessing Enhanced Care Management and Community Supports.​

What is involved?
Quarterly Convenings – Open to All
Learn alongside peers statewide at quarterly sessions focused on practical, scalable solutions for cross-sector data exchange.
Targeted Workgroups – Shape the Solution
Participate in monthly workgroups to co-design and test solutions for identifying shared clients, confirming ECM/CS eligibility, coordinating services, etc.. Get tailored technical assistance, peer learning, and early access to replicable tools.
