Opportunities for Homeless Systems of Care to Partner with Medi-Cal Managed Care Plans

This section of the CalAIM Toolkit for Continuums of Care (CoCs) and CoC Partners is intended to help counties, CoCs, and local homeless response system partners better understand what MCPs are likely to invest in and prioritize to address the housing and health needs of their Medi-Cal members experiencing homelessness.

These areas of opportunity and system alignment reflect areas of current and recent CalAIM investment, as well as areas where cross-system collaboration can significantly increase the effectiveness and impact of MCP efforts.

This handout helps answer the question: How can CoCs and their partners help MCPs address the health and health-related social needs of their members who are experiencing or at risk of homelessness, while also filling critical gaps in local homeless response efforts?
